The Care Transitions Market Leader is responsible for the promotion of quality, cost-effective services and care through a collaborative process that coordinates, monitors, and evaluates services provided to all transitional patients admitted to the nursing centers.

Position Highlights

Reports to Market PresidentIdentify practice patterns within the nursing centers that contribute to high resource utilization and monitor changes in status that could lead to hospital readmission and report to nursing. *Develop and implement effective discharge planning to improve both clinical outcomes and customer satisfaction. *Build and sustain market hospital partners and home health service agencies. *Utilize the appropriate National Population Health leader to grow strategic relationships and clinical programing.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification (full-time, part-time, or casual), job grade, location, and length of service.

Qualifications:

*RN License preferred but not required. *Prior experience in utilization review, case management, public/population health or discharge planning preferred. *Experience coaching clinical teams. *Preferred case management experience. *Experience in business/development preferred. *Excellent written, verbal and listening communication abilities. *Prior experience in utilization review or discharge planning preferred.
